Live Attenuated Vaccines Live attenuated vaccines have been evolving over time and include the commercially available varicella zoster virus7 and measles, mumps, and rubella M-M-RII ; vaccines. The benefit of live attenuated vaccines over other approaches is that the vaccines present the full range of antigens normally displayed by the wild-type virus as well as provoke a wide range of acquired immune responses.4, 7 However, these vaccines also present challenges, such as residual pathogenicity, reversion to virulence, the capacity to establish latent infections, and interference with serologic testing for HSV infection.2, 4 An early live attenuated genetically engineered vaccine, R7020, was found to be poorly immunogenic in humans, and development ceased.2, 18 Another live attenuated HSV-2 vaccine ICP10PK, developed by AuRx Inc. ; was more recently tested as an immunotherapeutic in patients with recurrent genital herpes. Patients receiving the vaccine were reported to have fewer recurrences of symptomatic disease compared with placebo.19 Data on the prophylactic use of this vaccine have not been published. Live replication-deficient or single-cycle HSV vaccines have evolved from earlier live attenuated vaccines. These vaccines release genetically mutated HSV-2 that either cannot complete a full round of replication, or that exit after an initial cycle of reproduction but cannot enter neighboring cells to continue the process of infection. Thus, these vaccines stimulate antibody response after immunization; some also establish latency, but cannot reactivate.4 One such vaccine is the DISC Disabled Infectious Single Cycle ; vaccine. This was found to be immunogenic in humans but was ineffective as an immunotherapeutic agent in persons with recurrent genital herpes.2 The DISC vaccine has not been tested as a prophylactic agent.
